In order to receive certification, please allow 4-6 weeks for processing, if mailing in your renewal. Please allow 2-3 weeks if paying online and faxing in your renewal form. The PDF files

3. To mail or fax your application: Please note that you cannot fill out the PDF form online. You must print the form.

• PRINT and complete correct application or recertification form
• Attach appropriate documentation to verify your credentials
• Mail your check or money order along with application to:

• Or fax application and all documents to our secure fax line
• Payment can be either sent by check or money order or you can pay online.

TO SEND A FAX INSTRUCTIONS: To send a fax, get your fax machine set up and ready. Dial 800-710-1539. When the machine answers your call, enter extension 1 and immediately start sending your fax.  If this does not work with your fax machine, just dial the above number, immediately start sending your fax. If you have any problems please call 800-710-1539 ext 1 and we'll be happy to help you.

Purpose of Payment

APPLICATIONS AND FEES

Please enclose $25.00 for all past due recertifications. You must download Adobe Acrobat Reader to read or print the following applications.

Note 1: If you wish to order a new or replace a lost or damaged certificate issued through the NTCB, please send $25 along with your request.

Note 2: You must contact the NTCB within 3 months of applying if your certificate did not arrive. Those inquiring after 3 months are requested to pay a $25.00 service fee. It is the responsibility of the member to keep the NTCB informed of their current mailing address and other contact information.

Note 3: If you are renewing your certification after expiration, you must enclose an additional $25 reinstatement fee in addition to the certification fee ($90). You will be renewed with your current expiration date. If you are 1 year or more past due (or more than 6 months past due and wish a new renewal date), please enclose an additional $65 ($130).

Re-Certification Guidelines

The NTCB no longer requires that members send documentation of CEUs. As long as applicants sign the form being submitted, list courses and CEU awards received and indicate on the re-certification form they will keep proof of course completion and CEU awards for two (2) years, the NTCB is not required to request nor keep on file any supporting physical documentation. 20 CEUs are required for recertification in your modality. If you don't have 20 CEUs:

• A minimum of 10 CEU must be in your primary modality -- in studies, seminars or workshops, anatomy & physiology, clinical skills, protocols, research, coaching or professional ethics. 
  
  
• A maximum of 10 CEU may be earned in healthcare, alternative healthcare, or self-improvement courses, seminars or workshops intended to improve the clinical skills and business practices of the professional.
  
  These guidelines have been adopted by The Higher Learning Commission and endorsed by the U.S. DOE. The Natural Therapies Accreditation Commission (NTAC) adopted these guidelines on March 27, 2009.
